Following are the 2008-2009 Online Student Emergency & Information Update Forms

 

Forms to be completed for ALL 1st to 5th grade students

Forms to be completed for Preschool students

To complete each form simply:

  1. Click on the form name
  2. A dialog box will open, click on Save (Note: you have to save the document on your computer before making any changes)
  3. A second dialog box will open, click on Open
  4. Enter the requested information using the tab key to traverse through the fields
  5. Save your changes
  6. Print the form
  7. SIGN and DATE each form as required.  We must have your signature to comply with school mandates.
  8. Return your completed, signed, and dated forms to the FASSV Office by May 16th, 2008.
